Getting People Back Into Life
Our goal is to help patients return to an independent
life as soon as possible. A continuing care plan begins the day of admission
and builds throughout the hospitalization in preparation for returning the patient
to the community. The family plays a vital role in all phases of care, from
hospitalization to recovery and continuing care. Through our aftercare plan,
the patient's treatment team develops a plan for the patient after he or she
leaves the program.
Admissions and insurance referrals from physicians,
human service agencies, the community and family are welcome, as are self-referrals.
Assessments may be requested by a referral source and are provided by qualified,
professional members of our clinical team. The need for admission is determined
by a physician.
Services for patients admitted are covered by Medicare
and most major health insurance companies.

the program is an important part of that community support as we effectively
treat the unique emotional and healthcare needs encountered by older adults.
