Getting People Back Into Life

Our goal is to help patients return to an independent life as soon as possible. A continuing care plan begins the day of admission and builds throughout the hospitalization in preparation for returning the patient to the community. The family plays a vital role in all phases of care, from hospitalization to recovery and continuing care. Through our aftercare plan, the patient's treatment team develops a plan for the patient after he or she leaves the program.

Admissions and insurance referrals from physicians, human service agencies, the community and family are welcome, as are self-referrals. Assessments may be requested by a referral source and are provided by qualified, professional members of our clinical team. The need for admission is determined by a physician.

Services for patients admitted are covered by Medicare and most major health insurance companies.
