Senior Horizons
The Family Support System
The problems of aging effect not only patients,
but families as well. One of the hardest realities a family faces is accepting
that a loved one has deteriorating health and needs professional help. Having to
intervene and seek treatment for a loved one can bring about feelings of guilt
and pain that affect the entire family.
An essential program component of Senior Horizons
addresses the needs and emotions of family members. We encourage them to talk
about their concerns. We want the family members to understand they are not
alone.
Additionally, we often include family members in
their loved one's treatment to enable them to better understand the emotional
and physical problems the loved one is experiencing.
Making Patients Feel At Home
The 14-bed facility offers an intimate, comfortable
environment for patient treatment and recovery. The private patient rooms are
spacious and designed to provide a pleasant atmosphere, while keeping in mind
the special needs of older adults.
To better treat patients, we bring together an
interdisciplinary team of healthcare professional including the primary care
physician, psychiatrists, psychiatric nurses, social workers, physical and
occupational therapists and other trained professionals. These professionals
share the goal of helping each patient achieve a more satisfying lifestyle.
Each patient receives a complete physical and psychiatric
evaluation upon admission. Then, an individualized treatment plan is developed
by the interdisciplinary treatment team which addresses both medical and emotional
needs through comprehensive medical care and structured daily activities. Specialists
such as recreational therapist contribute to the patient's improvement as well.
This individualized treatment plan is used as the guideline to measure the patient's
improvement through their stay at Senior Horizons.
