Mercy Transitional Care Unit
The Mercy Transitional Care Unit is a bridge from
acute care to a less intense level of care. It is a place where the patient's
needs can be met after the acute phase of the illness has been treated. MTCU
is recommended when a patient is not ill enough to require acute care, but may
not be well enough to go home. The patient can be more independent and perform
more self-care and the level of activity is increased.
The goal of the Mercy Transitional Care Unit is to provide a positive healing
environment for patients and promotes independence and endurance in activities
of daily living, confidence in self care practices and enhancement of health.
The specialized care team meets twice a week to discuss the patient's plan of
care. These healthcare professionals include profesionals from many different
areas of the hospital.
