Health Information Management/Medical Record Services
The Health Information Management
(HIM) Department at St. Joseph’s is located on the first floor of the hospital
and consists of coding, record processing, transcription and release of information.
The department exists to promote excellence in patient care by storing, maintaining
and retrieving healthcare information in a timely and efficient manner. Department
hours of operation for public access are 8 a.m. to 4:30 p.m. Monday through
Friday.
HIM is responsible for responding
to all requests for patient access to their medical records. Copies of records
will be released to patients who present a written, signed authorization request
form and furnish identifying information – social security number, date of birth
and photo identification – either in person or by U.S. mail. The forms are available
at the HIM front desk and patients also need to provide our staff with their
date of medical service. Requests are processed by the HIM staff, or the contracted
correspondence service, and a reasonable fee charged to offset costs associated
with requests for copies of records. Postage, including express mail costs when
incurred at the request of the authorizing party, shall be included. Walk-in
requests for records will be processed as soon as possible, but there may be
a wait of up to three working days.
