Outpatient Services

The Diabetes Self-Management Program is designed to help individuals with diabetes develop and continuously practice the self-management skills that will enhance and prolong the quality of their lives by reducing the risks for diabetes related complications. All patients are encouraged to begin the program with participation in a Group Assessment and Introduction to Behavior Change Session. An individualized plan is then established with input from the physician, patient and the Center’s clinicians. In accordance with the plan, the patient will attend a series of group classes which include the 10 Survival Skills content areas outlined by the American Diabetes Association. Some of these offerings are:


Basic nutrition and meal planning
Blood glucose monitoring
Medication management
Chronic and acute complications
Exercise
Foot Care
Sick-day management

This year-long educational program includes follow-up counseling at 3, 6 and 12 months to measure your progress towards the goals you have set and to give you on-going support for daily management of your diabetes.

Classes are held Tuesday through Friday from 8-11 a.m. and 12:30 p.m.-3:30 p.m. – completion of the group classes involves attending two 3-hour classes; a 6 hour class is available per request. These classes are taught by a certified diabetes educator and a clinical dietitian.

Individual counseling is available upon request for persons who are hearing impaired, have a language barrier or require one-to-one teaching. These may include:

Insulin administration
Diabetes counseling in pregnancy
Advanced nutrition
Stress management
Pediatric consultation

Also offered is a monthly pre-diabetes class that is for persons whose blood sugar levels are higher than normal, but not high enough to be considered diabetic. Research has shown that persons who adopt a healthy lifestyle, which includes healthy meal planning, exercise, and managing their weight, can delay or prevent diabetes.

A weight loss program, Health-Fit, is also offered. This class is designed to help people "fit" weight loss into their lifestyle. Research shows that if you are overweight, losing as little as 10-15 pounds (5-7%) of your body weight can help you either manage your diabetes or prevent diabetes from developing.

To participate in our outpatient education program, please obtain a physician referral or contact our office at (501) 622-4665 or (501) 922-5451 extension 4665. Educational consultation is reimbursed by Medicare and most commercial plans.


A Diabetic Support Group meets monthly. Guest speakers present on a variety of topics that are specific for the diabetic patient.
